Octagon Box by Akio Kamei (also names "Pile of Disks"), Bits and Pieces #08-G7330. This one required a repair before I could solve it: there's a small plate inside that was manufactured with very soft wood, and a metal pin fell against it and wore a channel in it (before I owned it, of course!). This made the cover fall off with no effort. I replaced the wooden plate with a much harder wood and the puzzle appears to work fine now.
